GI & Hepatobiliary Interventions

Image-guided procedures for biliary, liver and gastrointestinal conditions

GI and hepatobiliary interventions use image guidance to diagnose and treat selected conditions involving the liver, bile ducts and gastrointestinal system. These procedures can provide minimally invasive options when drainage, stenting or targeted vascular treatment is required.

Dr. Yugal Goyal's interventional practice includes PTBD, biliary stenting, liver abscess drainage and gastrointestinal bleeding embolization, along with other specialised hepatobiliary and gastrointestinal interventions.

Biliary Interventions

Percutaneous Transhepatic Biliary Drainage (PTBD)

PTBD is a minimally invasive procedure used to drain bile when the normal flow through the bile ducts is blocked. A catheter is placed through the skin and liver into the bile ducts using imaging guidance.

It may be considered in selected cases of obstructive jaundice or biliary obstruction when biliary drainage is required.

Biliary Stenting

A biliary stent is placed to help keep a narrowed or blocked bile duct open and allow bile to flow more freely.

Stenting may be performed as part of the management of selected biliary obstructions, depending on the underlying cause and the patient's condition.

Liver Abscess Drainage

A liver abscess is a collection of infected fluid within the liver. In selected cases, image-guided percutaneous drainage can be used to remove the collection.

Using imaging guidance, a catheter can be positioned within the abscess to allow controlled drainage while avoiding a larger surgical approach.

GI Bleed Angio-Embolization

Gastrointestinal bleeding can sometimes require urgent intervention when the source of bleeding needs to be identified and controlled.

Angio-embolization uses a catheter to reach the blood vessel responsible for the bleeding and deliver materials that help stop or reduce blood flow to the affected area.

TIPS

Transjugular Intrahepatic Portosystemic Shunt (TIPS) is an interventional procedure used to create a channel between blood vessels within the liver.

It may be considered in selected patients with complications related to severe portal hypertension, including certain cases of refractory ascites or variceal bleeding.

Enteric Feeding Support

Some patients who cannot take adequate nutrition by mouth may require enteral feeding access.

Percutaneous feeding tube placement, including PEG or PRG, provides a route for delivering nutrition directly into the stomach when clinically appropriate.

Why Image Guidance Matters

These procedures require careful planning and precise placement of needles, wires, catheters or other devices. Imaging guidance allows the interventional radiologist to identify the target area and monitor the procedure during treatment.

The appropriate procedure depends on the patient's diagnosis, imaging findings, symptoms and overall medical condition.

Frequently Asked Questions

Q. What is PTBD?

PTBD is a minimally invasive procedure that uses image guidance to place a drainage catheter into the bile ducts when normal bile flow is obstructed.

A biliary stent may be used to help keep a narrowed or blocked bile duct open and improve bile drainage in selected patients.

In selected cases, an abscess can be drained through a small access point using imaging guidance and a drainage catheter.

It is a catheter-based procedure used to control certain gastrointestinal bleeding by delivering embolization material to the blood vessel responsible for the bleeding.

TIPS is a procedure that creates a channel within the liver to redirect blood flow and reduce pressure in the portal venous system. It is used only in selected clinical situations.
